All students must meet the secondary school literacy graduation requirement to earn their high school diploma. For most students, this means passing the Ontario Secondary School Literacy Test (OSSLT). Students who do not successfully complete the OSSLT have other opportunities to meet the literacy graduation requirement. Students can contact their school principal to find out about these options.
For schools wishing to give the OSSLT or any EQAO assessments: For EQAO to provide your private school access to the OSSLT or any of the EQAO assessments (Assessments of Reading, Writing and Mathematics, Primary and Junior Divisions and the Grade 9 Assessment of Mathematics), your school must agree to this attestation. This attestation ensures the reliable and valid administration of the EQAO assessments in accordance with EQAO policies and administration guidelines.
Attestation
By clicking “I Accept,” as the representative of the private school, I attest to the following:
- that all school staff and test administrators will carry out all activities required to administer the OSSLT or any of the EQAO assessments with honesty and good faith and will comply with the provisions of this attestation as well as follow the user guides for the administration of the assessments.
- that all technical requirements have been reviewed, agreed to and implemented as required by EQAO (failure to do so will result in the school forfeiting its opportunity to administer the OSSLT or any of the EQAO assessments).
- that all school staff involved in the administration of the OSSLT or any of the EQAO assessments will read and adhere to the “Professional Responsibilities” for the administration of the OSSLT or any of the EQAO assessments.
- that any suspected or proven unethical conduct by students, teachers, school staff or any person performing activities related to the OSSLT or any of the EQAO assessments may be reported by EQAO to the Private Schools and International Education Unit at the Ministry of Education, or to the Ontario College of Teachers (if applicable).
- that the school will co-operate fully with any investigation that may be initiated by EQAO regarding the matters attested to in this attestation, including providing all relevant documents and materials requested by EQAO.
- that in circumstances of suspected or proven misconduct, including any acts of dishonesty by students, teachers, invigilators, school staff, scribes or any person performing activities related to the administration of the OSSLT or any of the EQAO assessments, the school will fully co-operate with an EQAO investigation (further details provided in the Agreement signed by the private school).
- that the OSSLT or any of the EQAO assessments will be administered at the physical school location as registered with EQAO and will be invigilated by a teacher, invigilator or principal registered to the school.
- that the school will not engage in or permit any type of misconduct before, during or after the OSSLT or assessment session, including but not limited to
- impersonating a student.
- breaching any security arrangements for the assessment session.
- providing false or misleading information.
- failing to follow the instructions for administration.
- giving assistance during the assessment session and allowing students to assist each other during the assessment session.
- enabling the use of prohibited aids (e.g., notes, mobile devices, smart glasses, watches or any additional technological aid deemed by EQAO as not suitable for the OSSLT or any of the EQAO assessments).
- using unauthorized software and/or tampering with a computer (including required EQAO software) during the OSSLT or any assessment session.
- allowing students to leave the assessment room without permission.
- acting in a manner that is intentionally disruptive, offensive or abusive before, during or after the assessment session.
- any breach or suspected breach of security of an assessment is reported to EQAO immediately
- that if a student’s response to a question on an assessment or questionnaire is deemed offensive, the principal may be notified.
- that the school will respond to a privacy breach as follows:
- Note: A privacy breach occurs when personal information is deliberately or inadvertently collected, retained, used or disclosed in ways that are not in accordance with the Freedom of Information and Protection of Privacy Act (FIPPA). The following are examples of unauthorized collection, use or disclosure:
- information collected in error
- information used for a purpose not consistent with the original purpose of collection
- lost or misplaced personal information
- stolen personal information (laptops, data drives or disks)
- accidental disclosure of personal information to an unauthorized person or group
- deliberate disclosure of personal information to an unauthorized person or group (for fraudulent or other purposes)
- The school will ensure that the breach is
- contained and assessed as soon as it is identified.
- reported to EQAO’s Director of Corporate and Public Affairs, EQAO, 2 Carlton Street, Suite 1200, Toronto ON M5B 2M9; Tel.: 1-888-327-7377. Information to be included is as follows:
- the date of the incident
- the location of the incident
- the date the incident was discovered
- how the incident was discovered
- the details of the incident (what happened)
- the actions or steps that the school took in response to the breach (e.g., how the school contained the breach; the persons notified)
- The school will co-operate fully with EQAO and will follow any direction from EQAO leading to the fulfillment of EQAO’s policies and procedures related to privacy breaches.
Non-Disclosure Agreement
By clicking “I Accept,” the school agrees to ensure that teachers, invigilators, scribes, school staff or any person performing activities related to the OSSLT or any EQAO assessments will comply with the following provisions:
- not to disclose in digital, written, verbal or any other form—whether before, during or
after the assessment session—any portion of the OSSLT or any EQAO assessment materials, including their content and/or responses.
- not to copy, save, reproduce, distribute, publish or post to the Internet (including social media) or anywhere else OSSLT’s or any EQAO assessment materials, including but not limited to photographs of the OSSLT or of any EQAO assessment or descriptions of the OSSLT’s (or any EQAO assessment’s) content (including but not limited to questions, instructions, images, examples, reading passages out loud).
to destroy all authorized materials used to complete the OSSLT or any EQAO assessment (e.g., paper notes) immediately after the assessment session.
